How do I get an adaptation?

In order to get an adaptation you may need to be assessed by an Occupational Therapist (OT). This is to make sure we provide what you need. To request an OT assessment please contact our Customer Services Centre on 01354 660789 to complete a telephone assessment. Once you have completed a telephone assessment we will aim to contact you within ten days to explain the next steps. In most cases an OT will visit to assess your needs.
